The Managing Diabetes Education Program consists of 4 class sessions that are 2 hours in length and presented once a week for 4 weeks. The Mini Class Series is an intensive program that consists of 3 class sessions that are 2 hours in length and presented once a week for 3 weeks.
Participants are encouraged to attend a one hour follow up class approx. 2 months after completing either the regular 4 week or 3 week intensive program.
Class 1: The Game Plan
Topics covered include basic information regarding diabetes including types of diabetes, how diagnosed, symptoms and risk factors. Lifestyle changes that can assist in controlling diabetes such as healthy eating and activity are also discussed. Goal setting/problem solving is reviewed.
Class 2: Eating Well for Life
Topics covered include balanced eating, effect of foods and fiber on blood glucose, timing/composition of meals and snacks, label reading, supermarket shopping, healthy meal preparation, portion control, carb counting, eating out guidelines and weight management. Goals will be reviewed and revised, as appropriate.
Class 3: Staying Healthy
Topics covered include blood glucose monitoring technique review, goals and guidelines for checking blood glucose, understanding blood glucose results, and pattern management. Symptoms, prevention and treatment of hypoglycemia and hyperglycemia will be reviewed. Additional topics include sick day rules, and prevention and treatment of long term complications. Goals will be reviewed and revised, as appropriate.
Class 4: Steps to Success
Topics covered include diabetes medications, social and family support, stress and relaxation, changing eating habits, problem solving, goal setting and resources diabetes self-management support resources.
