Cardiac Rehab - Holy Family Hospital

Holy Family Hospital is home to one of the Merrimack Valley's only cardiac rehabilitation programs. Our highly skilled staff and patient-centered approach have earned the distinction of program certification by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). 

You can benefit from cardiac rehabilitation if you have had:  

  • Heart disease such as angina or a heart attack 
  • Coronary bypass surgery or a balloon catheter procedure with stent inserted in your heart artery 
  • Heart valve surgery 
  • Heart transplant or heart failure 
  • Or if you have multiple risk factors for heart disease

Our Program 

Our interdisciplinary team of specialists uses a combination of state-of-the-art monitoring and individual education and counseling to support you. These caring professionals will help you learn how to regain a healthy, active lifestyle and help you adjust to these necessary changes. The staff is under the direction of a board-certified cardiologist and includes cardiovascular nurses, exercise physiologists, a registered dietitian and a licensed social worker.  We will work together with you and your referring physician to develop your treatment plan.

Cardiac rehabilitation typically begins four to six weeks following a recent cardiac event, and may continue for up to three months, depending upon individual needs and goals.  Sessions are offered three times per week (Mondays, Wednesdays and Thursdays), providing supervised exercise, education and social support.  Early morning and evening hours are available for your convenience. Cardiac rehabilitation is covered by most insurance plans. 

After “graduation” from the program, Holy Family Hospital also offers a self-pay maintenance program where you may continue your exercise in a medically supervised setting. 

According to program outcome measures, Holy Family’s cardiac rehabilitation program consistently demonstrates excellence in patient satisfaction and significant improvements in exercise tolerance and quality of life. 

Developing Your Personalized Treatment Plan  

Upon referral to Holy Family’s cardiac rehab program, our team will work with you to conduct a comprehensive evaluation, and develop an education and treatment plan tailored to your needs and goals. 

  • Cardiac Risk Profile – Cardiac nurses will identify your individual risk factors such as smoking, high blood pressure, high cholesterol, sedentary lifestyle, obesity and diabetes and work with you to develop both short- and long-term goals. 
  • Exercise Training – Exercise physiologists will teach you how to exercise safely and improve your stamina and strength, incorporating your individual ability as well as the results of your stress test.  Our staff closely monitors each exercise session assessing your personal response to exercise.  Your “exercise prescription” is approved by our medical director and guides your exercise progression. 
  • Heart-Healthy Education – Weekly one-hour educational sessions are available covering a wide range of topics such as coronary artery disease, risk factors, exercise, diabetes and stress management.  A library of texts, videos and DVDs are available for your use. 
  • Nutrition Education – A registered dietitian will teach you how to modify your eating habits and apply heart-healthy eating tips to home and restaurant dining. Appropriate caloric intake and diet composition will also be discussed. 
  • Stress Management – Our staff will help you identify and manage your sources of stress and their affect on your heart.  A licensed social worker is available for individual consultation.  Group classes are provided on Stress Management and Psychosocial Aspects of Heart Disease.  A facilitated support group is also offered several times throughout the year.
